Illness Meal Request Form

If you happen to fall ill this semester and would like a friend to pick up a meal from your dining hall using your Binghamton University ID card you must fill out a form.
The form can be found here.
After filling out, press submit, and it will take you to a page that explains what you need to do in order to successfully make the meal purchase. Make sure to print out the resulting form after you fill it out and give it to the person picking up your meal. The form needs to be presented to the unit manager at the dining hall you intend to make the purchase.
